What Is Community Medicaid?
In New York, “Community Medicaid” is a term commonly used for Medicaid coverage for people who live at home. It can provide standard health coverage — such as doctor visits, hospital care, and prescription drugs — and, for individuals who meet medical and functional eligibility requirements, it may also help cover long‑term care services in the community, including certain home care services. For home care, services and hours (if approved) depend on a needs assessment and the specific program or plan you're enrolled in. While eligibility and service levels depend on assessed need and program enrollment, Community Medicaid can help you stay in your home while receiving care.
Who Qualifies for Community Medicaid?
To qualify for Community Medicaid in New York, applicants generally must be in a non-modified adjusted gross income eligibility category (such as age 65+ or certified disabled) and meet both income and asset rules. In 2026, the commonly referenced income allowance is about $1,836 per month for an individual and $2,489 per month for a married couple (both applying). Nevertheless, excess income can be sheltered with a pooled income trust (see below).
The resource limit is $33,038 for an individual and $44,796 for a married couple (both applying). If income or resources are above these levels, eligibility may still be possible by gifting or setting up a Medicaid Asset Protection Trust (MAPT) for excess resources. There is currently no look-back period for home care. Keep in mind that Medicaid eligibility rules can vary by category and individual circumstances.
Strategies to Qualify for Community Medicaid
Whether you exceed the income limit or the asset limit for Community Medicaid, you can still take advantage of a few strategies to qualify.
Pooled Income Trusts
A pooled income trust works well for certain individuals who want to qualify for Community Medicaid but earn more than eligibility limits. This special type of trust is established and managed by a nonprofit organization (NPO). It exists specifically to supplement the needs of elderly and disabled individuals beyond government benefits.
With a pooled income trust, an individual can spend down their income while paying their nonmedical bills. The recipient submits their monthly bills, like rent, mortgage, utilities and other household expenses, to the administrator. The trust then pays those bills up to the amount they contributed.
There are several NPOs that provide pooled trusts, and typical costs involve startup fees and reasonable maintenance fees. When the participant passes away, their remaining balance often stays in the pooled trust to benefit other participants.

Medicaid Asset Protection Trusts (MAPTs)
MAPTs are irrevocable trusts used in Medicaid planning to help reduce countable assets and preserve assets for loved ones. When properly structured and funded, assets transferred to the trust are generally no longer treated as the applicant's countable resources, though the applicant typically gives up direct access to the trust principal. The applicant may elect to keep the income or not. Principal may be accessed by gifting virtually any amount tax-free to the heirs.
